The Women's Health Clinic is looking for a Registered Nurse who will serve as a champion for veterans as the Women's Health Nurse Care Coordinator (WHCC). The WHCC has a crucial role in assisting women veterans to efficiently obtain gender specific services and care, with emphasis on maternity care coordination, coordination of gynecologic services, and back up mammogram coordination and follow-up. The WHCC influences care outcomes by collaborating with members of the interdisciplinary team. The WHCC works closely with the pelvic floor physical therapist to obtain the skills and knowledge to be able to apply techniques learned in a clinical setting. Provides clinically appropriate treatment programs for client evaluation and treatment within the Women's Health Clinic to include testing and measurement of results to determine diagnosis, treatment, prognosis, and prevention. The WHCC will provide clinically appropriate treatment programs for client evaluation and treatment in the Women's Health Clinic. The WHCC will work closely with the Women's Health Medical Director, the Women's Health Program Manager and the nurse manager in coordinating and tracking gender-specific care (i.e. maternity care, mammography, gynecological coordination, etc.) of patients throughout all aspects of the medical center, including Care in the Community and other community agencies. Duties include, but not limited to: Ability to observe, identify, and respond to the patient needs for care, including medication, equipment assisted care and patient/family education. Demonstrates appropriate and consistent critical thinking skills evidenced by assessing, planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care. Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care issues. Promotes wellness by empowering the patient to self-management through education and patient driven health care delivery. Collaborates with patients through motivational interviewing to assess and identify needs, issues, care, goals, and resources in critical factors for achievement of desired outcomes for health maintenance and wellness. Treats all Veterans and families with dignity and respect. Listens to and honors Veteran and family perspectives and choices. Veteran and family knowledge, values, beliefs, and cultural backgrounds are incorporated into the planning and delivery of care in a non-judgment, non-discriminatory manner. Communicate and interact appropriately and courteously with all internal and external customers Maintain confidentiality of electronic, written, and/or verbal patient/employee information Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment Acts as patient advocate Performs other related duties as assigned.
