Providence Medical Group along with Collabria Care/Providence Home & Community Care is seeking a full-time nurse practitioner with experience or training in Palliative Care to join a well-established hospice team in Napa, California. This role involves providing comprehensive palliative care services needs across a variety of settings including, home visits, skilled nursing and long-term care facilities. This is a highly autonomous and flexible role where you will have the ability to choose your own schedule while managing your own patient panel. You will have access to an extensive support and resource network as well including an interdisciplinary team comprised of a social worker, chaplain, RNs, and community health workers.
