The RN practices in the Geriatrics/Palliative and Extended Care (GEC) Service and may be assigned to a clinic located in SLCVAMC catchment area. S/he uses the nursing process to collaborate with the Teamlet (including the Patient, GEC Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (Family/Caregiver, internal and Community-Based services involved in providing care to the patient) in developing the patient-driven care plan.
