The Primary Care Nurse Care Manager will provide care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process. The RN's primary commitment is to the patient, whether an individual, family, group, or community. The RN will be required to conduct face to face appointments, phone appointments, and virtual care appointments to execute the above duties. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
