Primary Responsibilities:
- Talks directly with members, telephonic screening/assessment in order to offer members the most approximate level of care which can assist them to remove barriers and address social determinants of care gaps
- Assess for clinical criteria for multi programs offered by Optum as well as by our client and their external vendors
- Determining the appropriate program offering and obtaining consent for programs such as CM, Chronic, MSK, BH, Palliative, Maternity
- Also includes assisting member with care coordination (for members who do not meet program criteria or decline) assist them with items such as appropriate resources, client wellness portal, etc. (Educate member regarding immediate care advice and preventative behaviors)
- 100% remote
- Most calls delivered through dialer
- All calls monitored for quality purposes
- Our nurses use soft phones and are logged into phone system

Required Qualifications:
- Active and unrestricted RN license in California
- 2+ years of clinical experience as an RN
- 1+ years of experience with call center nursing, telephonic triage, and/or case management
- Experience using a dialer system for outbound calls
- Must be willing to work an evening schedule and weekend rotation as follows:
- 10:30a - 7p OR 11a - 7:30p PST
- AND on average 1 Saturday every 3 months
Preferred Qualifications:
- Case Management certification/CCM
- Compact Nursing license
- Demonstrate exemplary nursing process skills and prioritization of symptoms described by member
- Outstanding telephonic phone skills with ability to assess, discuss and recommend interventions based on member need
- Bilingual Spanish
- Excellent written communication Skills. Documentation is accurate, timely, relevant, reflecting clear picture of member’s status/needs
