Job Description Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance. Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care. Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions. Collect, assess, and evaluate patient health data to develop and implement care plans. Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols.
